Intra cytoplasmic sperm injection is an invitrofertilization procedure in which a single sperm is injected directly into an egg. This procedure is most commonly used to overcome male infertility problems, although it may also be used where eggs cannot easily be penetrated by sperm, and occasionally as a method of in vitro fertilization, especially that associated with sperm donation.

Procedure – Before giving Intra cytoplasmic sperm injection to men, the woman will be given fertility drugs to stimulate her ovaries to develop several mature eggs for fertilisation. To screen for possible genetic problems that could affect offspring, experts recommend that men with little or no sperm in their semen have genetic testing before they proceed with ICSI.

• Ovulation and egg retrieval. To prepare for an assisted reproductive procedure using eggs the female patient gets daily injections and is closely monitored for 2 weeks before egg retrieval. The female patient is injected with gonadotropin or follicle stimulating hormone to stimulate the ovaries to produce multiple eggs. After the first week, the gynecologist checks the blood estrogen levels and uses ultrasound to see whether eggs are maturing in the follicles.

During the second week, the dosage may change based on test results and ultrasound. If follicles fully develop, the female patient is given a human chorionic gonadotropin injection to stimulate the follicles to mature. The mature eggs are collected 34 to 36 hours later using laparoscopy or needle aspiration guided by ultrasound through the abdomen to the ovaries.

• Sperm injection and transfer - After giving the woman a local anaesthetic, the doctor will remove her eggs using a fine, hollow needle. Under high-power magnification, a glass tool is used to hold an egg in place. A microscopic glass tube containing sperm is used to penetrate and deposit one sperm into the egg.

Two days later the fertilised eggs become balls of cells called embryos. After culturing in the laboratory overnight, eggs are checked for evidence of fertilization. The doctor transplants one or two embryos into the woman's uterus through her cervix using a thin catheter. A maximum of three embryos can be transferred if the woman is over 40 years old and is using her own eggs, one or two if she is using donor eggs. Extra embryos, if there are any, may be frozen in case this cycle isn't successful. One embryo may attach to the uterine wall and continue to grow. After about two weeks, the woman can take a pregnancy test.
